Supporting general practice post COVID-19

Brisbane South PHN Person-Centred Care Practices Program (PCCPP) sought to do a review of the PCCPP. Prestantia Health conducted a review of Phase 1 of the program to provide recommendations to the PHN to inform the development of PCCPP Round 2. Building on the recommendations Prestantia Health was invited to work with the PHN to co-design PCCPP Round 2, ensuring it was fit for purpose and addressed regional capability building needs whilst accommodating issues and challenges with the system.
